New updated Sussex wound care formulary:

The following nurse bases are now to order wound care products via ONPOS (delivery partners: NHS Supply Chain and North West Ostomy Supplies)

Brighton & Hove and Crawley, Horsham and Mid-Sussex 

Active nurse bases: TVN, Community/District Nurse, Practice Nurse, Nursing Homes and Specialist clinics (including Children’s Community Services and Leg Ulcer Clinics)

Coastal West Sussex and East Sussex

(High Weald, Lewes and Havens, Eastbourne, Hailsham and Seaford, Hastings and Rother)

Active nurse bases: TVN, Community/District Nurse, Practice Nurse, and Specialist clinics (including Children’s Community Services and Leg Ulcer Clinics)

Nursing homes in Coastal West Sussex and East Sussex

Formulary items should continue to be ordered from the patient’s GP using ‘Dressings Formulary Request Form 2025 - Nursing Homes’

If after undergoing a full wound assessment, there is a clinical need for a restricted (purple) formulary item or a non-formulary item, authorisation is required from a TVN or other specialist as identified in ‘Specialist list of products

The specialist will then complete and send you either ‘Dressings Formulary Request Form Specialist 2025 - Nursing Homes’ or ‘Dressings Non-Formulary Request Form’ which can be sent to the prescriber.
