Sussex wound care formulary:

Following the rollout of wound care direct supply across Sussex, nurse bases (TVN, Community/District Nurse, Practice Nurse, Nursing Homes and Specialist clinics - including Children’s Community Services and Leg Ulcer Clinics) in Brighton & Hove, East Sussex (High Weald, Lewes and Havens, Eastbourne, Hailsham and Seaford, Hastings and Rother) and West Sussex (Crawley, Horsham and Mid-Sussex and Coastal West Sussex) are now able to order formulary wound care products via ONPOS (delivery partners: NHS Supply Chain and North West Ostomy Supplies)

If after undergoing a full wound assessment, there is a clinical need for a non-formulary item, authorisation is required from a TVN or other specialist as identified in ‘Specialist list of products’  The specialist will then complete ‘Dressings Non-Formulary Request Form’ which can be sent to the prescriber.
