Gender Incongruence / Gender Dysphoria
In March 2024, NHSE published their commissioning position for puberty suppressing hormones (PSH).
PSHs are synthetic (man-made) hormones that suppress the hormones naturally produced by the body and in doing so, suppress puberty, with the aim of reducing the level of puberty-related anxiety in an individual with gender incongruence.
PSHs are not available as a routine commissioning treatment option for treatment of children and young people who have gender incongruence/dysphoria.
NHSE have concluded that there is insufficient evidence to support the safety or clinical effectiveness of PSH to make these treatments routinely available at this time.
Tavistock via a Shared Care Protocol (SCP)
The Tavistock and Portman NHS Foundation Trust are a gender identity clinic that provides national transgender healthcare for trans and non-binary people.
The service operates under a shared care model, which is a formal agreement that enables primary care clinicians to accept responsibility for the prescribing and monitoring of specialist medicines.
It is expected that this SCP will also be used for people under the care of a private specialist.
Sussex Gender Service (SGS) via a Collaborative Care Agreement (CCA)
The Sussex Gender Service (SGS) pilot is a waiting-list initiative; a local centre to serve the needs of trans and non-binary people in Sussex.
The service follows the same service specification from NHS England as other gender service pilots, which is similar to the existing gender identity clinics.
The SGS work under a collaborative care agreement (CCA), which allows for the prescribing and titrating of medication and the associated phlebotomy to be undertaken at patients' GP surgery.
